Children’s Hospital Colorado Specialty Clinics for both Neurology and Orthopedic/Rehab occur in our Sterling and Fort Morgan offices. These clinics are hosted regionally to reduce the amount of driving families have to do to access care at the Children’s main campus in Aurora, Colorado. Families from all across our region and some from Wyoming and Nebraska attend the clinics. Information about upcoming clinics is shared via postcards sent to existing families, local providers, community resource agencies, and on our website.
Neurology clinics occur every month, alternating between Sterling and Fort Morgan. The Orthopedic/Rehab clinics are currently on hold for 2026 due to provider availability. Those families who were/are receiving Orthopedic/Rehab services can still do so at Children’s Hospital Aurora campus or the Broomfield campus, with the intent to return to NE Colorado in 2027. We want to maintain contact with those families and reassure them that this change is only temporary.
New patients are always welcome. In order to be seen at one of the specialty clinics a referral needs to be made by their primary care provider. Once that referral is made, families are able to call and schedule their appointment directly with Children’s Hospital. A helpful tip when scheduling is for the family to state they wish to be scheduled at the Northeast Colorado Health Department’s outreach clinic in either Sterling or Fort Morgan. If they have any trouble they can contact me to assist with the process.
NCHD’s Specialty clinics are one part of the umbrella for the Healthcare Program for Children and Youth with Special Needs, also known as HCP. The HCP program serves children and youth from birth to 21 years. NCHD’s HCP team is composed of a coordinator and two nurses. The goal of the HCP team is to help improve the health, development and well-being of children and youth by promoting communication between families, providers and community resources. Through HCP, families can be connected to the care they need and be provided with information that can address a child’s behavioral, developmental, educational, financial, medical and/or social needs. Through care coordination, HCP assists Northeast Colorado families in understanding and coordinating the resources available for care and support.
Through care coordination our team can:
- Connect families with available resources, such as Early Intervention, WIC, immunizations, utility assistance, food assistance, childcare resources, etc.
- Provide assistance with locating and applying for appropriate insurance programs.
- Ensure that all children born in our region are screened for hearing loss and there is follow-up care.
- Provide nutrition education and screening by a registered dietitian.
- Provide social and familial screenings by a licensed clinical social worker to provide referral to treatment.
- Facilitate access to medical care by addressing a family’s individual case and barriers they may face, for example identifying providers for eye and dental care, transportation, or language barriers.
