The Community Health Assessment includes both quantitative and qualitative data indicators and information related to the health of our communities. The Community Assessment was used to develop the Public Health Improvement Plan. This Public Health Improvement Plan will provide strategic direction toward the best public health services to the residents of northeast Colorado over the next five years.
Quantitative Data
My Sidewalk Dashboard includes quantitative data on demographics, birth characteristics, maternal/child health, adult health, cancer incidence and death, behavioral risk factors, and other mortality data. In addition, there are links to a drug overdose assessment, report on STI rates, and data from a substance and opioid use assessment conducted in our 6 county health region.

Opioid and Substance Use Assessment
This report includes health indicators from publicly available data descriptors as well as data on indicators collected from regional partners including hospitals, coroner offices, and sheriff offices. The goal of this report is to provide a high-level assessment of opioid and other substance use, and relevant health indicators for the six counties served by Northeast Colorado Health Department (NCHD): Logan, Morgan, Phillips, Sedgwick, Washington, and Yuma.
Opioid and Substance Assessment Survey Results
The Northeast Colorado Health Department (NCHD) partnered with OMNI Institute to conduct an opioid and substance use regional assessment of Health Statistics Region 1 (HSR1), the catchment area which NCHD serves. In conjunction with a secondary data scan, a survey was distributed to key partners in HSR1 to gain a deeper understanding of the landscape of opioid and substance use issues and opportunities for prevention in Northeast Colorado. This document serves to highlight the main themes and lessons learned from this survey.
