The Diabetes Prevention Program Community Health Specialist will be trained as a Lifestyle Coach implementing a CDC-approved curriculum designed for preventing or delaying type 2 diabetes. Successful applicants should be passionate about community health and helping people live the best quality of life they can, with excellent communication and interpersonal skills, and be comfortable presenting in a small classroom environment to adults over 18.

Fluency in Spanish is a plus. Travel throughout the six-county region (Logan, Morgan, Phillips, Sedgwick, Washington, and Yuma) is expected.

Northeast Colorado Health Department is seeking a Community Health Specialist working in Tobacco Prevention, who focuses on reducing tobacco use and exposure by engaging at-risk populations through education, outreach, and policy support. They deliver prevention and cessation presentations to at-risk youth, work with residents and property managers in multi-unit housing to address secondhand smoke exposure, and collaborate with local partners to support and implement tobacco-related policies that promote healthier environments.

This position’s duties entail cold-call outreach within the community clinics and doctor offices, to promote our cessation offerings. A background in sales or marketing would be highly desired.
