Our Equity and Outreach division is dedicated to providing crucial assistance to community members and their families. We help them access and navigate the healthcare system, essential community services and resources, while also promoting healthy behaviors. Our multicultural and multilingual Community Health Worker team builds trust by serving as a liaison, providing culturally and linguistically appropriate services. Through partnerships and collaborations with other agencies, this team passionately takes an integrated approach to community outreach and support. We promote, maintain, and improve health outcomes by offering social support, health insurance literacy, community referrals, and public assistance application support. Additionally, we host and attend community events as well as advocate for individual and community well-being, all while giving back to our communities. Our team forges connections in locations where community members are comfortable and convene the most.

Language Services

The Community Health Workers (CHWs) on the Equity and Outreach team speak six languages other than English among them and are available to help in the community with translation for community events, assistance with forms, and accessing other regional services. Assistance is available in Spanish, Somali, French, Kinyarwanda, Kirundi, and Swahili. If you have questions or need interpretation, please call 970-867-4918.

Direct Services – Bridging the Gap

Health insurance through Connect for Colorado – The Community Health Workers (CHWs) with NCHD are certified Assisters that can guide people through the Colorado health insurance marketplace, the OmniSalud program that provides affordable health coverage for undocumented people, and help people better understand their health insurance options.

Public assistance program applications – CHWs can assist with application for programs such as Medicaid, CHP+, SNAP, and others.

Certified Car Seat Inspections available in English and Spanish, with interpretation assistance available in other languages. For more information about safety seats and inspections, please visit our Child Passenger Safety webpage.

Resource allocation – CHWs make referrals to community resources.

Outreach

The NCHD Equity and Outreach team work closely with many regional organizations that provide different types of services and resources; we can connect you with those organizations and provide translation when needed to be able to fully access what is available.

Diaper Resources

Free diapers and wipes are available in all NCHD offices by appointment.

Sizes available from newborn through size 6, and pull-ups in sizes 2T/3T, 3T/4T, and 4T/5T.

All families are welcome; one person per household (the guardian of the child) can pick up one package of diapers and wipes every 2-3 weeks depending upon availability.
