NCHD would like to introduce our National Diabetes Prevention Program (NDPP). This program will benefit residents of Northeast Colorado by providing resources to those who are at risk of developing Type 2 Diabetes so that they may be proactive in reducing that risk and lead healthier lives.
Northeast Colorado has high rates of Type 2 Diabetes, and this can lead to a variety of poor outcomes, such as increased risk for heart disease, poor circulation, limb amputation, and worse.
By giving participants tools and information, it is the goal of NCHD to prevent Type 2 Diabetes from occuring and help our program graduates live healthy, happy, and Diabetes-free.
Year Long Lifecycle Change Program
Utilizing NCHD’s own certified Lifestyle Coaches, the Lifecycle Change Program will teach and encourage participants to make lifestyle changes and choices to reduce their risk of developing Type 2 Diabetes. This is accomplished through information and coaching around healthier eating, increased activity, heart health awareness, and other relevant topics.
This program is offered both in person and online to increase access. Though your healthcare provider may refer you to this program, a referral is not required to participate. Look at the checklist below to see if you are eligible.
Who is Eligible
- You are 18 years or older.
- You are diagnosed with prediabetes.
- You have a body mass index (BMI) of at least 25.
- You can have a history of gestational diabetes.
- You cannot be pregnant at the time of enrollment.
- You cannot be diagnosed with Type 1 or Type 2 Diabetes.
